Option 1

Registered at under 7 last season, please complete and return Form R for each player.

Option 2

New to the League, please complete, supply and return the following for each new player at the under 8 age group:

- Form F click here to download form F

- photocopy of birth certificate or passport (only for players new to your team this year, if they were registered to the League last season they will not need to provide this information again)

- 2 identical passport size photos 35mm x 45mm (1 photo must have the players name and date of birth on the reverse)

- SAE (stamped self addressed envelope) for return of SPL ID cards
