Please complete, supply and return the following for each player at the under 7 age group:

- photocopy of birth certificate or passport 

- 2 identical passport size photos 35mm x 45mm (1 photo must have the players name and date of birth on the reverse)

- SAE (stamped self addressed envelope) for return of SPL ID cards
